Cardiac Rehabilitation - Community Cardiac Rehabilitation encourages people affected by cardiac disease to achieve and maintain good physical and psychological health.

Children's Short Breaks - Home Nursing Team The Children’s Short Breaks Home Nursing Team provides short breaks for Norfolk families caring for children with chronic/complex health/nursing care needs.
City Reach City Reach provides healthcare services for people who find it difficult to visit mainstream GP services.
Colman Hospital Colman Hospital, in the heart of Norwich, is also known as our Centre for Specialist Rehabilitation Services (CCSRS). The site brings together a number of specialist services within a group of individual units which are staffed and managed by our professional and experienced teams.
Community Admissions Avoidance – West Norfolk The service will provide an assessment of need for patients and put in appropriate interventions to either avoid an admission into hospital, respond to a discharge, or provide intermediate care services.
Community Neurology Service The Community Neurology Team provides tailored intervention to people with complex needs, as a result of neurological conditions.
Continence Service The Continence Service offers a specialised team of nurses and physiotherapists dedicated to providing an assessment and treatment plan for adults who have bladder, bowel and or prolapse problems with or without incontinence.
COPD Nurse Patients in hospital who may require home oxygen on discharge can be seen by the service, and patients will be assessed ensuring they fit the oxygen criteria and if appropriate the oxygen nurse will order the appropriate oxygen for discharge.
Cranmer House The staff at Cranmer House work closely together to provide joined up care which maximises each patient's independence and rehabilitation.
