Overview

GPs, consultants, community nurses, and hospital nurses and any other allied health professionals may refer patients to the COPD Nursing Service or the Oxygen Nurse for the West Locality, based at the Queen Elizabeth Hospital.

Patients in hospital who may require home oxygen when they leave can be seen by the service. Patients will be assessed ensuring they meet the criteria and, if appropriate, the oxygen nurse will order the appropriate oxygen for when they are discharged.

Patients referred to the COPD team may be seen during their stay in hospital or at home in the community after leaving hospital. New referrals to the service are currently assessed at home on the first visit. Further visits may be offered, as necessary.

Patients already in contact with the COPD team may self-refer for advice/assessment and exacerbation management.

COPD Community Clinics are held fortnightly (Wednesday afternoons) between Downham Market Health Centre and Hunstanton Surgery. These are outreach clinics led by Respiratory Consultant (QEH) and supported by the Community COPD Nurse.

The service also educates patients, both in hospital and the community, about the use and risk of oxygen. 

Patients on Long Term Oxygen Therapy (LTOT) are followed up in the community, where they have a blood test performed, checking oxygen and carbon dioxide levels. Oxygen can then be altered accordingly.

The Oxygen Nurse will work very closely with the COPD Specialist Nurse, Heart Failure Specialist Nurses and Macmillan Nurses.

Patients can contact the service directly with any queries they have about their oxygen therapy.
